Public Records Requests

Columbia County Health System (CCHS) is committed to open and transparent government. As a public hospital district, we recognize the public’s right to access records that reflect the actions and operations of our organization. In accordance with the Washington State Public Records Act (RCW 42.56), almost all records maintained by CCHS are considered public records and are available for inspection and copying, unless specifically exempt by law to protect patient privacy, confidential health care information, or other sensitive data. Our goal is to provide the fullest assistance to requestors, ensure a timely and fair response, and balance the public’s right to know with the obligation to safeguard individual privacy and maintain the efficient operation of our health system

How to Request Public Records

To request public records, please complete and submit a Public Records Request Form. The form can be emailed, mailed, faxed, or delivered in person to our Public Records Officer:
